ABOUT JANET
Janet T. Smith, LCSW-R, ACSW, C-ASWCM, received her Masters of Social Work degree from Fordham University School of Social Services in New York City. She is a Licensed Clinical Social Worker and a member of the Aging Life Care Association® of Geriatric Care Managers. Ms. Smith is also a Member of The National Association of Social Work and the Association for Certified Social Workers. She is a Field Instructor of social work education, certified by Hunter College of the City University of New York.
For over 35 years, Ms. Smith has worked with older adults and disabled adults in health care, specifically sub-acute care rehabilitation and home care. She began her social work career as a direct care social worker, assisting individuals and families in coping with the impact of illness and disability.
Ms. Smith has specialized experience in client advocacy, developing a care plan, educating clients and families about available resources, including government and other entitlements, facilitating appropriate discharge planning for a successful transition back to community living on their behalf, providing education about government and private resources to meet their care needs and discharge planning for transiting back to community living. Ms. Smith specializes in supporting solo-agers in their long-term care planning.
Ms. Smith has served as a hospital and community liaison, a director of nursing home social work and admissions, and director of health care public relations. Ms. Smith has extensive knowledge of sub-acute care rehabilitation and home care. As a result of her broad experience, Ms. Smith is knowledgeable about current healthcare practices and is able to navigate its modern complexities in order to help clients access needed services.
